Are you a new patient?
Before your first appointment, you'll need to complete a patient registration form, either the child or adult patient history form, and read and complete two other forms.
You can either download the necessary forms from this website to complete at your convenience or arrive 35-45 minutes early to your appointment to fill the forms out in office.

Insurance Coverage Information
To determine your insurance benefits and eligibility, please contact your insurance carrier directly by calling the customer service telephone number located on the back of your insurance card. Please note that you are ultimately responsible for payment of all services that are not covered by your insurance plan. Please be reminded that plan participation does not necessarily guarantee payment. Benefits are subject to plan limitations and exclusions, as well as member eligibility at the time of service. It is very important to bring your current insurance card with you on each visit. Even though our clinics are out of network for ALL insurance plans, we will make every effort to bill your insurance provider. However, we cannot guarantee coverage of services by your insurance company. Please feel free to contact us if you have any further questions.
Questions about your bill?
Most billing questions are best handled by your insurance company. By the time you receive our statement, we have done most of what we can do to resolve any billing issues, so please contact your insurance company first.
